- Fill in TMnet Streamyx Application Form with Terms And Conditions.
- Supporting documents accompanying the customer's application for subscription of the Service shall be required to be submitted to tmnet together with the Application Form. The required supporting documents are as follows:
For Malaysian:
I . Individual - a copy of the individual's Identity card (both side) and phone bill.
II . Sole proprietor or partnership - a copy of either the Customer's business license, form D, B, or A;
III . Company - a copy of Form 9, Form 49, Form 13.
- Installation upon successful, customer will be charged RM173, which will appear in the first monthly bills.
- The 1st bill will include stamp duty(RM 10), installation(*RM88) and service activation fee(RM75) of RM173, monthly subscription and pro-rate of monthly subscription fee.
